Just FYI, I've been fighting with this thing for three days. Finally found a forum post that helped. When using a phone ensure its set to auto-calculate the time based on your local time zone. Mine was turned off (a result of a cruise I took last year) so the time was not perfectly synched. Once I changed this one setting the authenticator and account locked onto one another without a hitch.
