So I did some reading up and his glaive combo is actually a glaive attack, blast, glaive attack. So the tank picking up the boss is taunting and getting the puncture as a result. It also means that if we screw up too late on the taunt, not just too early, we could get double stacks of puncture.

Doesn't change how we tank him really, but at least I understand where the damage is coming from and can time some of my mitigation abilities more appropriately.
